Clinical Rotation: Baptist Beaches 327 hrs During Clinical Rotation of 14 weeks, I was able to provide MNT in a 122-bed hospital. Applied the Nutrition Care Process, created nutrition diagnosis statements, goals and interventions for clinical diagnosis. Held group nutrition education classes for Diabetes, COPD and Malnutrition in addition to holding counseling session for individuals with Diabetes and CVD. Assessed and conducted inpatient education on diabetes, heart failure, cardiovascular disease, CKD, dialysis, diverticulosis, malnutrition, cachexia, vitamin K and nutrition support. Assessed high risk patients in the ICU and recommended appropriate Tube feedings and TPN’s. Presented 2 clinical case studies to staff and carried out a departmental performance improvement project.
Specialty Rotation: Mayo Clinic ▪ Fresenius Medical Center; Dialysis ▪ JAX VA Clinic South Point ▪Abbot 272 hrs Continued clinical practice at Mayo Clinic assessed and applied Nutrition Care Process to high risk nutrition support patients in the ICU and CCU floors. Implemented Tube Feeding and TPN regimens; monitor for nutrient adequacy, and tolerance daily by evaluating patient labs, signs and symptoms. Assessed for malnutrition using nutrition focused physical exam model. Provided nutrition education and counseling in an outpatient setting to cancer patients with PEG placements, Ostomies and to malnourished patients with post gastric surgeries. Provided nutrition care to patients in the ALS clinic and educated on nutritional needs through the progression of disease. At Fresenius Medical Center, conducted assessments and examined bone density labs of 120 patients and made appropriate nutrition recommendations. Conducted individual nutrition education and counseling with renal patients, regarding their labs and the importance of monitoring potassium, phosphorus, protein, calcium and vitamin D as well as food sources. Completed assessments for dialysis patients at the 1 month, 90 day and 1-year benchmarks. Hosted a tabling event educating participants on increased protein supplement use. Provided continuous clinical and counseling practice at the VA using standard assessment and counseling techniques to malnourished Vets. Educated on the Food First method prior to supplementation and on blood glucose control using CHO Counting and the Plate Method. Held group nutrition education classes for weight loss, Diabetes management and substance abuse at the VA.
Community Rotation: WIC ▪ YMCA Johnson Center ▪ Elevated Nutrition and Wellness LLC 121 hrs Counseled patients on nutrition component pregnancy and breastfeeding; Assessed high risk pregnant patients; Created an educational workshop on Gestational Diabetes; Created bulletin boards; Assessed the YMCA population for nutritional needs; Created informational handouts; Hosted nutrition tabling events weekly; Held cooking demonstrations to introduce clients to new foods; Taught nutrition education lesson on MyPlate for children ages 6-11 years.
